IMMUNIZATION RECORDS & DENTAL CARE

Each child must present a written report from his/her physician concerning an updated report on all immunizations. The Federal Government is very strict in this matter and requires a summary report of these each year.  Your cooperation in this matter is essential since this assures all children that communicable diseases are not spread.

ALL STUDENTS ENTERING KINDERGARTEN OR FIRST GRADE FOR THE 2000-2001 SCHOOL YEAR MUST BE IMMUNIZED AGAINST HEPATITIS B. THESE STUDENTS WILL NEED TO EITHER HAVE COMPLETED OR BE ON SCHEDULE FOR THE THREE DOSE SERIES AS A PREREQUISITE TO ENTERING SCHOOL.  CHICKEN POX VACCINE IS ALSO REQUIRED.

ALL STUDENTS ENTERING 6TH GRADE IN INDIANA SCHOOLS AFTER THE FALL OF 1991 WILL BE REQUIRED TO HAVE RECEIVED AT LEAST TWO (2) DOSES OF MEASLES VACCINE. BOTH DOSES MUST HAVE BEEN ADMINISTERED AFTER THE FIRST BIRTHDAY AND THERE MUST HAVE BEEN AT LEAST 30 DAYS BETWEEN DOSES.

If proper medical forms and immunizations are not received by October 1, the student will be suspended from school until such time that these forms have been received.

Dental cards will be given out in spring each year and must be returned no later than the end of the third grading period. This gives ample time for appointments during the summer months and subsequent follow-up appointments.
